Where can I check or raise a concern?

Keep the treatment name, the person's name and the written terms. They help you describe the question clearly.

Where does a concern about a doctor go?

For a medical doctor, or MD, the Arizona Medical Board has complaint forms.

DO is another doctor licence type; it has its own board complaint path.

The Nursing Board takes complaints about possible breaches of nursing rules.

How can I check the pharmacy?

FDA, short for Food and Drug Administration, links to state tools for checking pharmacy licences.

A pharmacy based outside Arizona needed a permit from Arizona to ship medicine into the state, as of October 3, 2026.

The pharmacy's home state also had to allow it to operate there.

A permit is not proof that a custom drug meets all the rules.

What if the concern is about a facility or an ad?

Arizona's Health Department takes complaints about licensed health facilities.

For unfair or misleading practices, the Arizona Attorney General takes consumer complaints.

A complaint does not promise a refund or another result.

How much does a public record prove?

As of October 6, 2026, Arizona limited which kinds of board records were online.

An online search does not give the whole past record.

You can ask the board that licenses the person for public records of past actions on that person's licence.

The board can explain which records the law allows it to release. An online search and a records request have different limits.

Use the person's full name and licence number when asking. If something still does not make sense, ask the person responsible for your care to explain it.
